Dr. Dave's Note: Our ISP is having heartburn trying to process our Forms. Since our ISP is not an animal, I cannot treat it. So, until the ISP gets better, instead of the nice, beautiful Form that I created, we'll have to rely on old-fashioned e-mail.

I will reply to you within ONE WEEK of receiving a Consultation Within a Week Form, er, E-mail.

With Consultation Within a Week, your Form is attended AFTER any EXPRESS Consultations, but BEFORE any Free Consultation or General Questions!!!

In ALL cases, Dr. Dave's reply to your Consultation Within a Week will be within ONE week.

The steps to submitting this consultation are:

  • Start a Send Mail to Dr. Dave
  • Copy & Paste (into your e-mail) the following headings: ORDER NUMBER: NAME: AGE: BREED: SEX: MEDICAL, VACCINATION and PREVENTATIVE HISTORY: REASON FOR CONSULTATION:
  • Please provide the following information (each Heading on a separate line, please):
  • Our Patient's NAME
  • AGE (and please tell me years or months or weeks or days)
  • BREED (examples: German Shepard, Siamese, African Grey)
  • SEX (Male/Intact; Female/Intact; Male/Neutered; Female/Spayed; Unknown)
  • MEDICAL HISTORY ( All previous problems, treatments and outcomes. Also enter most current vaccinations and preventions (such as Brand of Heartworm Prevention, Advantage, etc.). Type in UNKNOWN, if Medical History is not known. )
  • REASON FOR CONSULTATION: ( Current problem (What YOU see), current diagnosis (What your VET sees) if any, test results, examination results, treatments and outcomes. The more you can tell me, the more accurate my consultation can be. )

  • Press Payment Processing for Consultation Within a Week
  • At the Online Store Site, Check Consultation Within a Week, then press Check-Out Now
  • Complete the payment processing making SURE you write down, or Copy your Order Number (If you forget or lose your Order Number, check your incoming e-mail....I send you a confirmation with your Order Number within just a few minutes of payment processing)
  • Once you've completed the payment processing, bring your E-mail back up
  • Type in, or PASTE, your ORDER NUMBER (Messages without an Order Number will be treated as Free Consultations, which are completed as quickly as possible, but can take up to 2 - 3 WEEKs, or longer, depending upon my schedule)
  • Send your E-mail
