EXPRESS Consultation

Forms will be picked up between 9:00 AM and 12 Noon (Central Time) Monday through Fridays (except on holidays). EXPRESS Consultations are picked up more often than these guaranteed times.
You will receive a reply stating your EXPRESS Consultation Form has been received and given to Dr. Dave within 2 hours of it's receipt.
Dr. Dave will reply to your EXPRESS Consultation Form within 24 hours of receipt. Many times Dr. Dave can reply much sooner than the 24 hours, and he will make every effort to reply as quickly as possible.
Please fill the Form out completely, providing all signs of the problem (What YOU see), results of examinations, test results, and treatments and outcomes (how well it worked) along with your questions.
If necessary, you will be provided with a FAX number to send copies of results, progress notes, etc.
Payment covers this ONE condition's consultation and Dr. Dave's research based on his experiences and utilizing extensive printed and internet sources.
You will NOT be charged for unlimited followups for this same problem or topic. Any followup will be treated as an EXPRESS Consultation.

Payment covers this ONE condition's consultation and Dr. Dave's research based on his experiences and utilizing extensive printed and internet sources. You will NOT be charged for up to TWO (2) followups for this same problem or topic. The followups will be treated as a Consultation Within a Week.
